It will be the digital code inside, rockstar are releasing a physical disc in December.

However, you wont be able to play the game until the 19th, the official release date. There is no early access program that has been announced, for any edition of the game.

On the 12th November, you are able to download the game on your console, however you will only be able to play it when it comes out on the 19th November. This allows you to have the game fully downloaded and ready for when the game is out instead of having to wait for it to download.
So you get to download it from the 12th but wont be able to actually play the game until the 19th
I think the 12th is for preloading as its apparently a 200 GB plus install, this gives you time over over the week to download it and to stop everyone doing it on the 18th and breaking the Internet. With most of the game pre installed you will probably own need to download a final tiny portion to allow gameplay on the 18th. This is justva guess so I could be completely wrong lol
No, you will not be able to play the game on November 12th. This date is only for retailers to hand out the box containing the code leaving you 7 days to pre install the game. Everyone will only be able to play the game from November 19th though. There is no early access or anything of that sort.
